A TEAM that provides a joined up holistic approach to community-based health and social care has moved to Denbigh Infirmary.

The Denbigh Community Resource Team (CRT) includes health staff such as community nurses, physiotherapists, occupational therapists and older person’s mental health teams, as well as social service workers and care practitioners.

They aim to provide care closer to home, offering better provision for Denbigh residents with a focus on prevention and early intervention.

It is understood the team will be based at the Llenewi Ward which closed following a fire safety review in 2017.

Other services will also link with the team at its new site, including carer assessors, voluntary organisations and GPs.

Denbigh CRT is one of the nine joint teams in Denbighshire and Conwy, a partnership between Denbighshire County Council, Conwy County Borough Council and Betsi Cadwaladr University Health Board.

Cllr Bobby Feeley, Denbighshire County Council’s cabinet member for well-being and independence, said: “The introduction of Community Resource Teams across Denbighshire has been a major step forward in terms of health and social care working together, providing an improved, seamless service so beneficial for those we care for.

“I am delighted another CRT is moving to one base at Denbigh Infirmary, allowing the team to work closer together across the different service areas.

“Being co-located provides many benefits for residents by improving service delivery and helps support a preventative approach to create resilient communities, which is a priority for the council.”

The team moved into Denbigh Infirmary having previously been based at Ruthin County Hall, Ruthin Hospital and Glan Clwyd Hospital.

Sally Macdonald, Denbigh CRT lead, said: “There are a lot of benefits to bringing together colleagues from different disciplines who all play a part in effective community-based care.

“Simply by being together and working from the same place, it helps improve communication and teamwork, which in turn will help us provide better, more joined-up care for our patients in the Denbigh area.”
